Cook the macaroni in plenty of salted water until al dente.
Melt the butter in a large saucepan and sauté the onion cubes until translucent. Scatter the flour on top and stir, cook for about 3 minutes. Add the milk, cream, salt and pepper and bring to the boil over medium heat, stirring constantly. Let simmer gently for 2 minutes. Remove from heat, stir in nutmeg, chilli and 2 teaspoons cheese. Then add the macaroni and mix in.
Put everything in a greased baking dish, sprinkle with the remaining cheese, spread the breadcrumbs on top and cover with flakes of butter.
Put in the oven preheated to 175 ° and bake for about 20-25 minutes.
